The install via CPAN assumes rt3. It fails for rt4:
perl -MCPAN -e 'install RT::Extension::MandatorySubject'
...
Cannot find the location of RT.pm that defines $RT::LocalPath in: inc

Path to your RT.pm:
And entering the path to RT.pm on my system just loops back to that
error.
