1. Bug: horizontal rules are converted to four dashes but not surrounded by
blank lines so they don't render as horizontal rules.
2. Feature: handle named anchors.
As we refine the email refactor stuff, I may have more of these. You can
tell me to shut up or that you will ignore them for a while.
