Hello,
On an Apache v2.4.41 on Ubuntu with mod_perl v2.0.11 I received the following error early on in Apache vhost error log:

After troubleshooting at length, I pin pointed the source to mod_perl not running well under mpm_event. Once I had switched to mpm_prefork, everything worked back normally.
So to reproduce install an instance of Apache with version as above with mpm_event.
SetHandler was set in the ghost to mod-eel
