Please see the attached patch against the latest version of Curses::Forms.
There is a bug in addSubform that causes forms to inherit the alternate
widget's namespace instead of using the alternate form namespace(s)
specified during initial configuration.
The bug only rears its head when there is SINGLE alternate form
namespace specified.
This needs to be fixed or things fail to work as expected.
